Person Directed Services (PDS) are home and community –based services that can help people of all ages, and all disabilities maintain independence and choose the supports that they need to live their best life.
People with disabilities and their families do not have to be on their own when it comes to person-directed services. Supports Brokers can assist with enhancing natural supports, employer-related responsibilities, and following program rules. Supports Brokers work in conjunction with the person, their family, but also the Supports Coordinator, and other service team members.
Supports Brokers may also assist a self–directed person with the following:
¨ Recruiting, hiring, scheduling and supervising Service Support Professionals (SSPs) ¨ Expanding and coordinating informal, unpaid resources and supports ¨ Completing required paperwork ¨ Keeping track of Person Directed Services (PDS) ¨ Facilitating a Circle of Support ¨ Developing and maintain back-up plans ¨ Enhancing person-person centered planning and communicating modifications to the service plan ¨ Fulfilling Common Law Employer or Managing Employer responsibilities ¨ Problem solving to help achieve outcomes ¨ Compliance with Waiver and Person Directed Services (PDS)
Supports Brokers must complete the Office of Developmental Programs Training and Certification program. Supports Brokers are trained in person centered planning, Charting the Life Course Tools, and Community Mapping. Each Supports Broker works independently and as part of a team to assist the person self-directing and/ or their surrogate to assist the person is living their best life, addressing their wants, needs and concerns and needs.
